PureVision Multifocal - Rebate Info

Like many other manufacturers of contact lenses, Bausch & Lomb is offering a mail-in rebate when you purchase Purevision Multifocal contacts. Although the rebate is not promoted on the B&L web site, it looks like some online retailers have special mail-in-rebate agreements - see below for more details.

For purchase made after January 1st 2010

At this time there is no mail-in rebate available.

For purchase made before January 1st 2010

If you buy 4 boxes (each box containing 6 lenses) of Purevision Multifocal, your rebate is $50.

If you buy 2 boxes (each box containing 6 lenses) of Purevision Multifocal, your rebate is $20.

The 2009 Purevision Multifocal rebate is available until December 31, 2009 to new or existing lens wearers after getting an eye exam and then purchasing Purevision Multifocal contacts.

How to receive the 2010 Purevision Multifocal Mail-in Rebate

Get an eye exam from your eye care practitioner.

Buy 2, 4 or more boxes of Purevision Multifocal lenses from a participating Eye Care Professional or online store.

Receive your mail-in certificate together with the purchased (and shipped) boxes.

Collect the following:

- 2 UPC Codes (they are printed on each contact lens box, so just cut them out).
- A copy of the receipt of your eye exam or fitting consultation
- The original product purchase receipt (copies are not accepted). The receipt must indicate the total number of boxes purchased.

Where to mail:

Your mail-in certificate should contain a one-time usable 16-digit personal code.  Go to www.bausch.com/rebates, enter the code and fill out any other additional information required. You should also see a mail-to address for your UPC Codes and your receipts.

Keep a copy of your paperwork for your records.

Which online stores are offering the Purevision Multifocal rebate?

Bausch & Lomb is currently not promoting this rebate on their site, so we were digging around on some of the popular contact lens retailer

sites and found the following merchants offering this rebate:

• 1800contacts.com - is offering a $40 rebate based on buying 4 boxes.

• searsopticalcontacts.com - even though not a huge player in the online retail market for contact lenses, searsopticalcontacts is one of the few sites offering a $20 rebate (when purchasing 2 boxes) and a $50 rebate (when purchasing 4 boxes of Purevision Multifocal lenses).

Additional Notes:

• Keep in mind that it can take up to 60 days for delivery of the rebate check.
• Bausch & Lomb will not accept submissions without an eye exam receipt.
• Bausch & Lomb will not accept copies of the UPC Codes.
• Make sure to check the expiry date of the mail-in rebate. Purchases must be made before December 31, 2009 and your mail must be postmarked before January 31, 2010.
• Product purchases must be made within 90 days of exam and submissions must be postmarked within 60 days of purchase.
• The offer is limited to one rebate per person and the offer is not valid in combination with any other offer or rebate.
